FOSTER/COMPANION CARE: An individual who is seeking foster or companion care as a residential option may either select a foster care setting provided by St. Giles or they may select a family member who can be trained and paid by St. Giles to provide this service. The foster/companion care provider provides direct personal assistance with the activities of daily living and with facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities and supervision of the individuals safety and security.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

GROUP HOME LIVING: If an individual is looking for a group home setting to serve his or her residential needs St. Giles offers three, four and six bed models. Each home is well furnished and provides a safe and secure environment based upon the needs of each individual resident.

INTERMEDIATE CARE FACILITES (ICF/MR) Intermediate Care Facilities for Persons with Mental Retardation (ICF/MR), is a Medicaid certified program, serving 6-13 individuals in a residential group home setting with a 24 hour shift staff. Individuals are evaluated and an individualized treatment plan is developed to assist each individual in functioning at their greatest ability.

Who is Eligible for ICF services? The Individual must have a diagnosis of mental retardation, be in need of and able to benefit from the active treatment provided in the 24 hour supervised residential setting and meet the Medicaid financial eligibility criteria and the Level of Care determination criteria

How do individuals enroll in ICF/MR? Enrolment can be sought through the local MRA or the family/individual may contact the program directly to inquire about the available openings.

Three and four bed models are available for individuals seeking HCS residential services. An individual who is seeking only supervised living may select a three bed model. Individuals seeking residential support will be directed toward the four bed model.

The three and four bed model offers most individuals his or her own bedroom. Trained staff are provided based upon the needs of the individual residents. Awake staff is provided in all six and four bed models.

Staff provides assistance with all daily living activities and training in self help and independent living skills.

SUPPORTED HOME LIVING: This option is for individuals who want to continue to live in their own home or the home of their natural or adoptive family members. St. Giles can provide trained staff that will coordinate and provide direct personal assistance with the activities of daily living and coordination of community activities and skill development toward greater independence.
