Intermediate Care Facility (ICF/MR)

 

Intermediate Care Facility for Persons with Mental Retardation (ICF/MR), is a Medicaid certified program, serving 6-13 individuals in a residential group home setting with 24-hour shift staff. Individuals are evaluated and an individualized treatment plan is developed to assist each individual in functioning at their greatest ability.

 

Who is eligible to receive ICF services?

 

The individual must have a diagnosis of mental retardation, be in need of and able to benefit from the active treatment provided in the 24-hour supervised setting and meet the Medicaid criteria and the Level of Care determination criteria.

 

How do individuals enroll in ICF/MR?

 

Enrollment can be sought through the local Mental Retardation Authority (MRA) or the family/individual may contact the program directly to inquire about available openings.

 

How are Support plans individualized?

 

Each individual receives a comprehensive functional assessment to determine strengths and needs. Individuals choose personal goals, which training is developed to meet.

 

What services are provided in ICF/MR?

 

Services include health care, day habilitation, supported employment, psychological, money management, and training to enhance daily living needs.
